If you are interested in meeting with us to discuss what you are going through, please print and fill out this Personal Data Inventory sheet. This helps us to have a full understanding of who you are, where you are coming from and what you are looking to accomplish.
After filling this out, either email it to Pastor Matt Tomlinson at mattt@mbcmi.org or drop it off at the church. We will take a few days to look it over, and we will give you a call for further information, if needed, or to schedule our first appointment together.
